To provide a superior level of access to testing services, Strata Diagnostics is in-network with most U.S. insurance plans, including, but not limited to, Aetna/Coventry, Cigna*, Humana, and UnitedHealthcare. In addition, we accept all major insurance plans.

Please send the completed form to the mailing address below or via fax to 617-401-4032, Attention: Client Services Results Requests. Please allow up to 30 days to receive a copy of your results. The items requested can either be picked up at StrataDx with photo identification, or delivered via FedEx with Direct Signature Required.
